Healthplex is known as "The Dental Plan Specialists." The company was established in the 1970's by dentists and other healthcare professionals who were concerned about the increasing costs of dental care. With an understanding of dentistry as well as dental insurance, Healthplex was able to develop benefit plans that effectively managed the risks associated with conventional indemnity plans. 1.8 million subscribers later, the corporation is providing dental benefits and administrative services for insurance companies, HMOs, school districts, municipalities, welfare funds and businesses of all sizes.

The plans available through Healthplex are underwritten by Dentcare Delivery Systems, Inc. in New York, and by International Healthcare Services, Inc. in New Jersey.

Employee Application
Please use the employee enrollment form to add a new employee to your plan. This form is also used when an employee is adding on a family member or changing their coverage. You can submit a new enrollment via fax or email.

Do you have an employee that is leaving the company and wants to keep their coverage? If so, please submit this application to us and we will extend the employee’s coverage per federal guidelines. Please note that the employee will stay on your bill, so you are responsible for billing and collecting premium for that person directly.
